Below are the forms used by District Administrators to review each site's SCIP in the spring.  Schools/programs are encouraged to print a copy of these forms to use as a self-assessment tool prior to signing and submitting the SCIP. (Reminder:  SCIPs need to be signed and submitted by the first Friday in May.)
